Accessibility Statement

We want everyone to be able to read this site, book a visit and take part in care. If something here does not work for you, tell us and we will fix it or find another way.

Our commitment

Cognitive Works is committed to making its website and telehealth services usable by people with disabilities, in keeping with the Americans with Disabilities Act and Section 1557 of the Affordable Care Act. We aim to meet the Web Content Accessibility Guidelines (WCAG) 2.1 at level AA.

How this site is built

Telehealth and communication

Video visits are held on a standard browser-based platform that supports captions. If you need an accommodation for a visit, such as a sign-language interpreter, large-print documents, extra time, or a telephone visit instead of video, tell us when you schedule or call 952-300-6277. Interpreters are provided at no cost to you.

Known limitations

Some documents in the onboarding packet are provided as PDF; if you need any of them in another format, we will provide it. Third-party services we link to (payment processing, electronic signature, video) have their own accessibility practices, which we review when we choose vendors.

Tell us

If you find a page or form that does not work with your assistive technology, or you need information in another format, contact contact@cognitiveworks.org or call 952-300-6277. Please tell us the page and what happened. We respond within two business days.
